A Comparison of the Cumulative Incidences of Metachronous Colorectal Adenoma and Cancer According to the Initial FindingsAmong Colonoscopically Followed-up Patients

ABSTRACT

The study included 2,740 patients who underwent follow-up colonoscopies (follow-up period 61.9 ± 37.7 months, fre-quency 3.3 ± 1.6 times). They were divided into five groups according to initial findings as follows: 633 with negative colonoscopy (group A), 721 with diminutive adenomas (group B), 622 with low-grade adenoma > 5 mm (group C), 307 with high-grade adenoma and intramucosal cancer (group D), and 457 with invasive cancer (group E). The cumulative incidences of patients with metachronous low-grade adenoma and those with high-grade adenoma and cancer (index lesion) both significantly increased step by step between groups A, B, C, and D by the log-rank test (p < 0.01, p < 0.05), although group E showed a significant low incidence of patients with low-grade adenoma. The incidences of patients having index lesion within 3 years and 5 years during follow-up were 1.9%, 1.7% in group A, 2.3%, 3.2% in group B, 3.2%, 3.8% in group C, 4.0%, 5.3% in group D, and 6.9%, 7.8% in group E, respectively. The suggested value of 4% as an acceptable incidence for the 5-year interval is suitable for those in group C, but not for those in group D, and the 3-year interval is suitable for those in group D, but not for those in group E.
