Abstract Obesity hypoventilation syndrome represents an embedded epidemic within the larger epidemic of obesity. Its prevalence, though largely unknown, seems much lower compared to OSA. Preoperative identification is key as most patients are largely undiagnosed at the time of surgery. Perioperative challenges include difficult intubation; failed airway; rapid desaturation predisposing to anoxic injury; associated comorbidities like heart failure and pulmonary hypertension and higher chances of opioid-induced ventilatory failure. Special preparedness is required on the part of the anesthesiologist and most of the management strategies are empiric.
