Heterogeneity across pancreatic β-cells is well known and there is growing appreciation that sub-populations of islet β-cells exist as transcriptionally (and functionally) distinct groups. Here, we discuss current evidence on heterogeneity and provide a model to recognise how β-cells might function together as a mini-organ, over a lifetime. Being mindful of these possibilities would aid in developing strategies to prevent, treat, or manage functional β-cell loss in diabetes.
