INTRODUCTION Besides physiological conditions caused by menopause, the quality of life of women in the transition to menopause is affected by other factors, including attitudes toward menopause and empowerment. Although some studies have evaluated the effect of these factors on quality of life, few studies have included all the factors in one study. Therefore, this study aimed to determine the relationship of quality of life with attitude towards menopause and women's empowerment during the transition to menopause in Gonabad, Iran. MATERIALS AND METHODS This cross-sectional study was performed on women during the menopause transition registered at the Comprehensive Healthcare Centers in Gonabad, Iran, from 2023 to 2024 based on simple random sampling. Demographic data, the Menopause Rating Scale (MRS), Menopause-specific Quality of Life (MENQOL), Women's Empowerment Status Questionnaire (WESQ), and Attitude Toward Menopause (ATM) questionnaires were used for data collection. Data were analyzed using the Pearson correlation coefficient and linear regression model. RESULTS A total of 626 participants (mean age: 53.88 (Standard Deviation (SD) =6.30 years) were evaluated. The mean WESQ, ATM, and MENQOL scores were 67.40 (SD =3.99), 63.54 (SD =3.31), and 3.86 (SD =3.99), respectively. Low and average levels of empowerment were observed in 50 (0.8 %) and 576 (92 %) participants, respectively. Negative attitude toward menopause was observed in 420 (67.1 %) participants, while good, moderate, and poor quality of life was observed in 334 (53.4 %), 199 (31.7 %), and 93 (14.9 %) of the participants, respectively. There was no significant correlation between MENQOL and WESQ (r=0.028, p=0.491) nor between MENQOL and ATM scores (r=0.036, p=0.368). The regression analysis confirmed the finding after adjusting potential confounding variables, too. CONCLUSIONS Attitude toward menopause and empowerment were not related to quality of life in this study.
