ABSTRACT

We have read with great interest the article entitled ‘‘Portal inflow modulation by somatostatin after major liver resection: A pilot study.’’ It is a very interesting topic because the relationship between high portal vein pressure (PVP) and ‘‘small for size’’ syndrome (SFSS) has been clearly demonstrated insomuch as some authors refer to it as ‘‘small for flow’’ syndrome. SFSS was first described in living donor liver transplantation, and it has been attributed to a PVP above 20 mm Hg with secondary sinusoidal endothelial cell injuries. The same pathogenic mechanism has been attributed to major liver resections with a small future liver remnant
