Microdialysis is a technique that may be used to directly investigate brain chemistry in-vivo. Although initially developed over 35 years ago (Ungerstedt and Pycock 1974), it is only relatively recently that studies have begun to utilise microdialysis in patients with glioma and other brain tumours. In this chapter we will review the general principles of microdialysis, the use of the technique to investigate glioma pathogenesis and evaluate chemoand radiotherapy, and the potential utilisation of retrograde microdialysis to administer chemotherapeutic agents directly to the tumour bed.
