Children and adults of short stature are particularly disadvantaged in the current regional allocation system, which is under active revision by the UNOS Liver and Intestinal Organ Transplantation Committee.1 The reasons for this persistent health disparity include national policy that prioritizes local adult over national pediatric candidates,2 discordance in donor-recipient size matching, 3 and other clinical considerations not accounted for in the MELD-Na score.4 Split-liver transplantation (SLT) presents a potential opportunity to address these disparities and help improve access to transplantation for children and adults of short stature.
