Azathioprine is one of the commonly used steroid-sparing immunosuppressive agents. Previous studies have looked at trending mean corpuscular volume (MCV) to measure therapeutic response to azathioprine which have suggested greater increase in MCV in patients with a good clinical response.\n Methods: \nRetrospective chart review was performed to collect data on azathioprine dosing, prednisone dose, MG ADL scale score, manual muscle testing (MMT) score, and MCV values. Nineteen patients were included in this study. Subjects were stratified into groups by their median azathioprine dose (mg/kg) (n): < 1.5 (6), 1.5-1.75 (4), 1.75-2.0 (2), 2.0-2.25 (3), > 2.25 (4). Pearson correlation coefficient and p-values were calculated for each patient and group.\n Results: \nMean age was 47.5 years. Average duration on azathioprine was 4.2 years. The correlation coefficient between change in prednisone dose and change in MCV was 0.28 (p=0.02), 0.28 (p=0.08), -0.74 (p<0.01), -0.57 (p<0.01), -0.74 (p<0.01), for each group respectively. The correlation coefficient between MG ADL and change in MCV was -0.44 (p<0.01), -0.09 (p=0.57), -0.77 (p<0.01), 0.16 (p=0.38), and -0.49 (p<0.01), respectively. The correlation coefficient between MMT and change in MCV was 0.47 (p<0.01), 0.37 (p=0.02), -0.83 (p<0.01), 0.14 (p=0.45), -0.45 (p<0.01) respectively.\n Conclusions: \nOur data shows that at an azathioprine dose of greater than 1.75 mg/kg, there is a significant correlation between increase in MCV value and reduction in prednisone dose and suggestive of improvement in clinical status.
