Abstract This chapter outlines the goals of therapy for the contemporary management of patients with heart failure with reduced ejection fraction (HFrEF). It provides an overview of general measures, including dietary and activity recommendations, and then focuses on diuretic use for symptom management and the foundational therapies to improve clinical outcomes in these patients. Emphasis is given on the rationale and pathophysiologic basis for different therapies and the effects on hemodynamics, ventricular structure, functional status, and clinical outcomes. Furthermore, clinical guidance is provided around side effects, complications, and drug interactions with these different therapies.
