Abstract Nature is a rich source of lead molecules for drug discovery and development. However, the irony is that despite this wealth, only a handful of molecules from the plant origin have reached the clinics. Although various phytochemicals have shown promising effects pre-clinically, most of them have failed during clinical trials due to poor oral bioavailability, safety concerns, and lack of pharmacodynamic reproducibility in clinical trials. In the present review, we have attempted to comprehensively describe the hurdles which hinder the successful clinical translation of phytochemicals. We have explicitly explained each obstacle and studied carefully the phytomolecules that have been clinically approved, thereby paving the way to improve the therapeutic translation of potent phytochemicals like withaferin A, resveratrol and curcumin. An interdisciplinary approach is the need of the hour for the development of future lead molecules of plant origin. The present review aims to discuss these aspects in detail.
